Clutch trouble!

Hello everyone.
I have yet another mechanic gremlin. I suspect a clutch failure as she slips in third/fourth gear especially up little hills as in the revs surge very high with no speed increase at anything above 35mph. The clutch grabs at the very tip top of the throw :/ as well. Someone previously cut a hole in the bottom of the bell housing right behind the pressure plate as well long ago so I am able to see the throwout moving and releasing when the pedal is pressed. The car has had this issue since I've owned it. I was wondering if anyone could shed some light on this issue for me thank you!
 
Wouldn’t call that premature. Depends a lot on the driver some are hard on clutches others not so. Might not be the original clutch.
 
Check all the pedal and clutch adjustments before jumping to the conclusion that the clutch has failed. If the drop arm on the release housing is fitted in the wrong position on the shaft then as the clutch wears the arm can hit the bellhousing and stop the clutch fully releasing, hence the slip.

The right honourable member "Vaultsman" had it happen on his 3500S when it still had the 4 speed fitted.
 
The hole in the bellhousing was likely done to recover the bits of failed bendix starter drive at one time or another.
