To be honest, it was very easy. Flatten the one you have go use as a template. Cut a new one out, bend it, drill a few holes.

Dear Chrisw, Thank you for your response. It is not the heater box complete that I am looking for, it is the left hand operating rod and fork that I need, please. I would be prepared to pay a reasonable amount for the part or make a donation to the forum. Whichever you think best. No need to dig out any pictures.
